Brookhaven Hospice is seeking a self-motivated and compassionate Registered Nurse – Admissions to join our growing team. As our Admissions Nurse, you’ll be the face of our organization in hospitals and inpatient facilities, educating, assessing, and supporting patients and caregivers while collaborating with hospital teams to provide timely and appropriate hospice services.

• Location: GIP Units with UMass Hospitals located in Southbridge and Worcester, MA
• Schedule: Full-Time, Monday - Friday
• Required: Active RN license & hospital bedside experience
• Preferred: Hospice or palliative care experience

• Serve as the clinical point of contact for hospice referrals and admissions in hospital and GIP settings
• Assess patient eligibility and complete clinical documentation and consents
• Provide compassionate education to patients and families about the hospice philosophy and services
• Build and maintain strong, trust-based relationships with hospital case managers, discharge planners, physicians, and facility staff
• Promote Brookhaven Hospice as the provider of choice through professional presence and service excellence
• Support census growth goals by identifying new opportunities for patient admissions

• Current, unencumbered RN license
• Minimum one (1) years of hospital nursing experience required
• Clinical confidence and a passion for patient-centered care
• Ability to balance clinical assessments with empathetic conversations
• Comfort working independently in fast-paced, hospital-based environments
• Sales/marketing experience in healthcare is a plus but not required

$95,000 - $105,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
